Internal NoteKeep getting "Fatal Error: Controller monitor failed. Controller not started" for the RAID card. This would cause the card to think that the raid bios was not installed. Was able to get into raid config after loading default settings for bios. However, after a reboot the message would come back up.
Even after the error didn't come back up and I was able to get to the installation of fedora, I got another error message when trying to create the partitions. "Host adapter reset request. SCSI hang ?" was the message that came up when I switched to the fourth console during partition setup.
Will need to RMA card. It's an Adaptec ASR-3405 (PCI-e x8). Left card in server so other noc can try and see if they have better luck. Will have to do text install of FC6 as display for install was fuzzy in graphics mode.

Internal NotePlease try different scsi cables and bypass backplane (if there is one). just rig it up for testing outside of the chassis if needed.

Internal NoteBooting the Controller Kernel......
Fatal Error: Controller monitor failed. Controller not started
Does not appear to be disk issue
Researched the error a bit and came up with this notice:

Most recent issues appear at top
Compatibility issue between Adaptec SATA RAID cards (2410SA, 2810SA...) and revision 1.1A Supermicro X6DAL series motherboards. When booting the system the 2810SA startup stays at "Booting the controller kernel......." for a long time then times out. Eventually this error comes up: "Fatal Error: Controller Monitor failed controller not started." To resolve this problem you must either change to non-Adaptec RAID card or change the motherboard to one with PCB revision 1.2.

Internal NoteRAID controller is booting every time now
Nothing changed so this may be a flaky card, or reseating it multiple times helped
installing OS now

Internal Noterebooted to add xfs to the kernel boot params and now the raid card is no longer posting again
FYI boot this: linux text xfs
graphics mode comes up very distorted
RAID error in detail:
Adaptec RAID BIOS V5.2-0 [Build 15323]
(c) 1998-2007 Adaptec, Inc. All Rights Reserved.
<<< Press <Ctrl><A> for Adaptec RAID Configuration Utility! >>>
Booting the Controller Kernel.......

Controller #00: Adaptec 3405 at PCI Slot:03, Bus:07, Dev:0E, Func:00
Booting the Controller Kernel.......
Fatal Error: Controller monitor failed. Controller not started
<< Press any key to continue ... >>
It takes 5-6 minutes to go from "Booting.." to "Fatal.."
After the second "Press any.." it will boot from CD but will not detect the drives. The installation hangs when it tries to load the aacraid driver.
However, the 8 or 9 times when this server booted successfully, nothing hang and I was able to select partitions to create on the array.

Hi,
We re-did the install formatting the XFS partition first this time and it no longer has the errors.
We also fixed the ram issue, so now 12GB is showing in the OS.
Login details are the same as the ones previously posted.

Hello,
we no more have I/O troubles using yum... but we still have this while hdparm tests:
"HDIO_DRIVE_CMD(null) (wait for flush complete) failed: Inappropriate ioctl for device"

This error looks like the libraries hdparm uses haven't may have not been updated for this controller.
Also keep in mind that hdparm is made for IDE devices. I see that the tests still come back with a value, so I think the hardware is good now and it is just a software issue.
The stress test you are running now should give you a good indication of what the machine can handle. There are no more I/O errors coming up in the system logs, the problem before was with the XFS partition that was created in the installer the first time.
Thank you,
