In the JS in your HTML page that has the eventhandlers you should have:

<SCRIPT>

</SCRIPT>

then in the eventhandler for SelectCal, pass in form and url:

call = SelectCal(form, url)

and for SwitchCal:  call = SwitchCal(url)

then see changes to the functions below:
