I have installed and configured LCDial, but it is not translating ${EXTEN} when referenced in the dial string on the provider table

Look:

Aug 10 03:00:35 WARNING[7220]: app_lcdial.c:197 lcdial_exec: LCDial: Got provider Number 1 called  with dialstr: IAX2/xxxxxx@yyyyyy/${EXTEN}
&#61550;	xxxxxx@yyyyyy/${EXTEN}

This is seen at debug in console, where it should have replaced the ${EXTEN}

Obviously, my provider returns this message when passed the symbols "${EXTEN}" when it should be receiving numbers (in my case) or a valid extension:

    -- IAX2/zzz.zzz.zzz.zzz:asdf-g is circuit-busy
Aug 10 03:00:36 WARNING[7211]: chan_iax2.c:6820 socket_read: Call rejected by zzz.zzz.zzz.zzz: No such context/extension
